Details - The direct quadraturerules for

simplex-3d order of 2,3 and 3,4
and
pyramid order of 1

are broken. Currently the quadraturerules are computed from other
rules, to avoid the problems. Especially for the simplex the rules
should be fixed, because the rules now are defined on a distorted
cube, which leads to a high number of quadrature points.
