[Dune] ALUGrid Ghost Entities

Hello Peter,

for a ALUGrid ghost element (entity codim=0) everything is implemented 
as for any other entity of codim 0, except two things:

1) the intersection iterator is not given the inside entity which is 
going to be fixed soon

2) the hierarchic iterator is not available and cannot be implemented
and for ghost element is not needed from my point of view since father 
children relation is not clear and so on
