Details - Now the 2.0 release is out (thanks again to Christian), we should decide when to remove the stuff that was deprecated in 2.0.

Moreover, the following questions arise (feel free to add more):
- Should we also remove dune/grid/common/referenceelements.hh (old referenceelement)?
- Should we remove the old MPI_CPPFLAGS / MPI_LIBS?
