Your gmsh-file contains quadrangles, indicated by the second tag '3' of elements 17-20. The GmshReader is not able to handle other than simplicial elements. Introducing some output (print elem number, number of tags and the tag id) to the GmshReader, I got the results in the attached file. It seems as if the last node, number 7, of elem 17 is interpreted as the number of tags for the next element in the GmshReader...

Btw, did you create and export this mesh via gmsh? I always thought gmsh constructs simplicial meshes only.
