[Dune] Semantics of standard IndexSets

Dear Dune,
while looking for a potential show-stopper for fixing fs#347
the following question arose:

Which entities are contained in Level/LeafIndexSets?

Unfortunately the documentation is incomplete for prallel grids.
The fact if ghost entities are contained is not documented.
Part I of the grid paper describes the index map in Definition 25.
However I can't find a definition of the set E with the 5 indices.
Especially the restriction to process p seems to be unclear.

Perhaps one of the authors can comment on this.
