[Dune] Removal of deprecated VTKOptions

Hi Markus,

I totally agree with you. I just want to point out again that the
deprecation was there for a long time but it didn't work. I think that
was the real problem. That is why everybody was surprised by the
removal. Then of course one has to find out what went wrong. But svn
diff can help here.

To your previous mail one could add, that it also doesn't help to
deprecate classes/methods when there is not a proper replacement
available, for example, boundaryId on class Intersection.
