OK, the make command line

make CPPFLAGS="-DALUGRID_SIMPLEX -DGRIDDIM=2" tester

did the trick. The gridtype.hh documentation is plain wrong.
With subsampling == 0 everything seems fine. For subsampling == 1 the
vertices of the original cells seem correct, while the additional vertices
seem to get the values of one of the origninal vertices instead of an
interpolated value. My guess is that the VTKFunction used to interpolated the
vertex data only gives correct values for the original vertices. Will
investigate further.
