[Dune] Physical entities read by GmshReader seem to "bleed out"

Hi Miguel,
can you reproduce the same effect with a different grid, say UGGrid?
can you reproduce the same effect with a smaller mesh?  A one-digit
number of elements would be a lot easier to debug.
